Here is where miswak either earns your trust or does not.
A 2025 systematic review published in Scientifica (Wiley) assessed miswak's antibiofilm properties across multiple randomized controlled trials. The researchers found a positive impact on controlling dental plaque, measured using the Modified Quigley-Hein and Loe-Silness plaque indices. These are standard clinical measurement tools, not subjective assessments.
A separate 2025 systematic review published through PMC/NIH, covering studies up to April 2024, reached a similar conclusion: miswak statistically significantly reduced plaque accumulation and gingival inflammation, performing comparably to or better than conventional toothbrushes.
In 2024, a randomized controlled trial at Princess Nourah University College of Dentistry compared miswak and conventional brushing over two weeks in 60 participants, evaluating outcomes using the Silness and Loe indices.
